SUBJECT: STANDARDIZATION OF PDOS CERTIFICATE
As part of OWWA’s effortS to standardize the implementation of the Pre-Departure Orientation Seminar Program, includinG the Certificate of Attendance (CA), all PDOS Providers are hereby advised that starting 01 October 2009, all CAs should conForm with the following specifications:
Size : 8.5″ X 5.5″
Color : Yellow for landbased workers
Blue for seafarers
The CA should reflect the following:
1. Complete name of worker with her/his middle name
2. Skill/Occupation of worker per employment contract
3. Country ol destination (landbased workers)
4. Philippine Recruitment AgencY
5. Foreign Principal
6. Foreign Employer
7. Date of PDOS
8. Certificate Number
g. Title of seminar at the center
10. Name of PDOS provider with its logo at the topmost center part of the certificate
11. Name and position/title of authorized agency official and his signature at the lower right hand corner
12. Name of PDOS Trainor and his signature at the lower left hand csrner
